Accelerator success: improving postnatal discharge at Chelsea and Westminster Hospital

Earlier this year, DigitalHealth.London Accelerator partner CW+, selected Accelerator company Lumeon (from our 2016-17 cohort) to help digitise its postnatal discharge process. They have now launched the innovative software, and here describe the difference it is making.

CW+, the charity of Chelsea and Westminster Hospital NHS Foundation Trust, have been working with Lumeon to create, test and embed the digital end-to-end care pathway for new mothers and their babies. The user-friendly technology is transforming how the postnatal department runs – streamlining processes, coordinating care towards a timely and safe discharge, and enhancing the experience for women and their families.

All activities and tasks are orchestrated and automated in real time, with the ability to get a good overview of the care being delivered. Digital dashboards allow multi-disciplinary team members to quickly see what they need to do next, and by when, for each patient to meet their discharge slot. Staff communicate more effectively with each other and new mums, to provide a more holistic overview relating to them and their baby as regards getting them ready for home.
