NHS Problem
50% of people entering reablement need some form of medication support. The current model provides medication-only care calls or support through reablement where elements of personal care are added to a care call to support multiple medication rounds a day. Medically unqualified social care workers are increasingly delegated the responsibility of both assisting with and administering medication in domiciliary care settings. Medication care calls cost the NHS over £13.4 million every year. Workforce shortages mean care packages are not made available resulting in patients staying longer in hospital. Medications account for 25% of the NHS Carbon footprint.
The Solution
Using YOURmeds to support 16,000 people with their medication management, could repurpose 1 million care calls and save £13.5 million each year. YOURmeds would also reduce the amount of medication waste and reduce the over-ordering of prescriptions. Patients could be discharged from hospitals more quickly reducing NHS costs. Our service users report increased independence and carers talk about a reduction in carer burden, family networks are better informed and have more peace of mind.
Impact
In an average trust, 6,800 people undergo major surgery each year. Complications and additional bed days cost approximately £68.4 million. Applying individualised prehabilitation could save £38.2 million, 15,640 bed days and reduce CO2 emissions by 659,150kg. These extra bed days could be used to increase elective capacity.
