Nawale Janati

Digital Cardiac Nurse Specialist

Biography: Nawale is a Digital Cardiac Nurse Specialist based at West Middlesex Hospital, and her work covers the AF Virtual Ward in collaboration with the Remote Monitoring Team and Lead Consultant with the aim of looking after, developing and reviewing a management plan for these patients. She also covers the Arrhythmia Clinic and she is working towards becoming an Arrhythmia Specialist Nurse. The hybrid approach she has been able to witness and practice while caring for patients is something that has always fascinated her. Before covering the role of Digital Cardiac Nurse Specialist, Nawale has worked and contributed to service improvements within the Cardiac Catheter Lab as part of a wider team. She is passionate about mental health and staff well-being and became a Mental Health First Aider for her department. Beyond her full-time job, she also produces social media content to support nurses finding fulfilment in their career and preventing burnout.

Horizon Fellowship project summary: It has been observed that a large number of the AEC AF Pathway referrals are lost, therefore patients don’t manage to be assessed by a specialist in a timely manner.
Audit data from the service show that there is a high rate of re-attendance to the emergency department when patients first present with AF. This is more likely if care is delayed. Nawale aims to explore digital solutions that can streamline the referral process, by creating a dedicated Cerner list for AEC AF Pathway referrals, where patients will be followed up by a specialist in a timely manner and receive the care they need.

Estimated number of patients / staff impacted by the project: Around 50+ patients

Goal(s) for time on the Horizon Fellowship programme: Nawale’s goal is to:
