My Health Guide helps learning-disabled adults take an active role in their health care, remain healthier and be better supported by carers. My Health Guide helps people manage their information, tell others how they’re feeling, and record their communications with friends, families, keyworkers and health professionals. Users experience better quality outcomes (reduced readmissions and hospital appointments, more stable daily patterns, more effective communications) and service providers are able to give better quality support (reduced repetition in sharing information, better retention, more consistent hand-over between agencies).
