Biography: Hannah is a Paediatric Neurodisability Consultant working within Cheyne Child Development Service. She is lead for Transition within her department and sits on the West London Childrens’ Healthcare steering group for Adolescent Healthcare Transition.
She has a keen interest and passion for finding solutions to complex problems. The increased use of technology amongst patients has driven her to look for digital solutions that can support information sharing and allow enhanced patient centred care. Areas of interest and service development include Transition to adult care, advance care planning and improving care for children with medical complexity.

She sits on the Executive Committee as Co-education Lead of The Colab Partnership, a group of like-minded individuals who are looking to improve healthcare for children with medical complexity and their families through education and service improvement.

Horizon Fellowship project summary: The project aims to explore and utilise digital tools to support Young People and their families during transition to adult services.
This includes:

  • Digital patient-healthcare interface for communication and information sharing that is specific to Transition
  • The ability to map out and individualise the Transition journey
  • Patient held record
  • The ability to share links to key resources with Young People
  • Patient/family involvement to ensure that solutions are centred around patient needs

Estimated number of patients/staff impacted by the project: Within Cheyne Child Development Service alone there are >30 young people with complex medical conditions on their Transition journey each year. This is a small cohort of the Young People that are transitioning to adult care within the Trust. The tools developed through this project will be applicable across West London Childrens’ Healthcare and has the potential to be applicable Nationally.

Goal(s) for time on the Horizon Fellowship programme: Hannah aims to find innovative ways to approach and tackle this project by exploring possible adaptations to tools that are currently in existence or the development of a new technological solution if needed.
