Long-term Conditions Programme at South West London Integrated Care System


Biography: In his current role, Daniel is responsible for driving digital innovation and transformation by leveraging his technological expertise, guidance and support. He actively contributes to the development and implementation of innovative digital solutions by managing digital projects and aligning them with ICS goals. Daniel also oversees the development of digital initiatives such as the self-management app and prevention portal, which are instrumental in transforming operations and audience engagement. He is dedicated to leading the team towards digital transformation by embracing cutting-edge technologies, streamlining processes, and bringing about positive change.

South West London ICL Digital Pioneer Fellowship project summary: Patients with chronic conditions (long-term conditions) spend less than one per cent of their time receiving NHS care. The rest of the time is used to self-manage their condition, which could be in a state of education with risks of deterioration. The South West London Integrated Care System have co-produced a free-to-use and locally configurable digital self-management app to support those self-managing long-term conditions. The primary aim is to make this self-management solution accessible to all, which will subsequently ease the burden on healthcare professionals and lower the occurrence and development of chronic illnesses.

Estimated number of patients / staff impacted by the project: Patients: 31,057 patients have been provided access to the app, with a total of 17,714 active users. Staff: This could potentially have an impact on all staff groups prescribing the app (e.g., 6908 repeat appointments or 169 GP days have been saved so far). Approximately 30 to 40 staff are involved in the app’s development, but this can significantly increase when clinical groups are involved in sign-offs for new modules.

Goal(s) for time on the South West London ICL Digital Pioneer Fellowship: Daniel aims to network, learn from and be mentored by like-minded professionals in order to develop the necessary skills and knowledge to lead health and care transformation projects, ultimately driving digital innovation in healthcare. He is also keen to advance his career in digital healthcare and grow professionally in his role as a digital project manager.
