Biography: Dalanya is a qualified Integrative Psychotherapist who uses a person-centred and trauma-informed approach in her work. She has vast experience working with victims of domestic violence, prisoners, rough sleepers with drug and alcohol issues, and managing a supported living home for adults with complex needs. As a Community Health and Wellbeing Champion, Dalanya’s role is jointly funded and requires her to ensure that the health and wellbeing needs of the community are met. She conducts extensive research to identify gaps in current services and introduces new services to meet those needs. Her primary duty is to encourage and support engagement with the available services and advocate for the residents of Pollards Hill to build sustainable services that meet their health and wellbeing needs.
South West London ICL Digital Pioneer Fellowship project summary: To assist and empower the local community by providing digital and social engagement that caters to their health and wellbeing needs. Data gathered from BPMs within the community is transferred and coded to connect with patients’ medical records for recommended follow-ups. This enables the community to self-monitor their health needs and connect with the appropriate services. The project aims to support and encourage the community to use BPMs and understand the procedures that follow, ensuring that every contact counts towards their overall health and wellbeing.
Estimated number of patients / staff impacted by the project: Patients: 20,000. Staff: 60.
Goal(s) for time on the South West London ICL Digital Pioneer Fellowship: Dalanya aims to grow and develop, enabling beneficial results in her professional career and meet the needs of the local community she serves. She is also keen to design and implement projects with clear and concise evaluations. Additionally, Dalanya hopes to build key networks and relationships to support partnership working and sustainability.
