Digital Pioneer: Whole Systems Integrated Care dashboard

The Whole Systems Integrated Care dashboard programme (WSIC) is an innovative tool from the North West London Collaboration of Clinical Commissioning. It has been designed to improve data sharing and interoperability.
The programme will generate a suite of tools to improve direct care for patients, including a Health and Social Care Integrated patient record. All health and social care providers involved in a patient’s treatment can review these WSIC Dashboards, using role-based secure access protocols.
“The WSIC dashboard will collect, link and cost patient identifiable data from ten Acute, two Mental Health and two Community Trusts across eight CCGs; and Social Care data from eight boroughs and 380 GP practices,” explains team member Ian Riley, “To date, this enormously rich data has already been used to create a Health and Social Care Integrated patient record and a number of long-term condition-specific watch list and radars.”
Further work on the project is also being undertaken to aid the management of long term conditions, including a chronic obstructive pulmonary disease radar and a population health dashboard to support emerging Accountable Care Partnerships.
