CW Innovation Fellowship project summary: My project aims to assist the Acute Provider’s collaborative with the digitalisation of the Fast Track pathway for rapidly deteriorating patients.

 Estimated number of patients / staff impacted by the project: Tens of thousands going forward.

 Goal(s) for time on the Horizon Fellowship programme:  To create short Vimeo content on the discharge pathway and guidance on how to complete the forms.
