Digital Cardiac Specialist Nurse

Biography: Cindy supports and contributes to the organisation virtual ward programme adhering to protocols and guidance agreed by the Team’s consultant cardiologists, basing decisions on professional knowledge and expertise. She provides expert assessment of patients with chest pain, to operate within protocols, utilising advanced nursing skills and knowledge to identify patient suitable for monitoring patient on the virtual ward. Cindy promotes safe and effective discharge of patient, and promotes care pathways for virtual ward for chest pain and arrhythmia. She works closely with the Northwest London (NWL) hub to ensure policies and procedures are adhere in accordance patient safety.

Horizon Fellowship project summary: One of the Trust priorities is to enhanced patient care and management through digital innovation and one of them is the virtual ward. There has been some evidence that patient can be optimally managed through their own environment being monitored remotely with trained clinical staff. This also promote patient’s well-being by preventing them occurring hospital acquired infection. The virtual ward also promotes decrease length of stay and help to create bed capacity.

Estimated number of patients / staff impacted by the project: We aim to admit 20% of cardiac patients admitted (considered low risk) on chest pain virtual ward. For staff we aim to target 50% of A&E staff to train about the service.

Goal(s) for time on the Horizon Fellowship programme: To gain knowledge and learn new skills and techniques on how initiate the project that will enhance staff awareness about this new initiative in Cardiology. Learn and adapt systematic approach, such as formulating a business case, appropriate person to contact, how to involve stakeholders and other.
