Biography: Anca is a motivated Programme Manager with demonstrated expertise in both social care and health best practices and processes. She is highly committed to improving and maintaining the quality of health and social care services through collaboration and partnership working. Anca is a strategic influencer with proven experience working with senior staff across multiple partner agencies to identify and achieve shared goals. In the Digital Pioneer Fellowship programme, her objective is to enhance integrated neighbourhood services by developing a data-sharing framework that facilitates effective risk stratification and targeted interventions. She will use her strategic leadership and collaborative skills to drive cross-sector partnerships, aligning with K&R priorities. Anca will focus on creating scalable and sustainable digital solutions that lead to measurable improvements in health and care outcomes. Through the fellowship, she aims to refine my skills further and contribute to innovative digital solutions.
South West London ICL Digital Pioneer Fellowship project summary:
The aim of the digital programme in K& R is to connect people and digital systems across Kingston and Richmond to achieve a “digital backbone” that improves health and care services for the local population.
There are three digital areas of focus for K&R for 24/25.
- Develop a digital solution focused on Integrated neighbourhoods
- Support the development of a risk stratification tool that promotes both Prevention and Health improvement agendas
- Promote an environment where digital opportunities play a key role in enhancing greater connectivity for staff and local communities (Connected Richmond)
The first area of focus that we aim to finalise in the next 4 months is Connected Richmond (which is a digital platform that will support the social prescribers` work). There is a similar platform in Kingston.
The aims of connected Richmond are to:
- Identify the functions of Connected Kingston that should be replicated in Richmond
- Determine the features that would enhance the usability of the digital platform
- Assess which local information is currently difficult to find and would be beneficial to include *gather specific accessibility requirements that should be addressed on the platform
- Explore effective strategies to promote the digital platform
Estimated number of patients / staff impacted by the project: Because we are starting in a phased way we can approximate 200 patients in 3-4 months.
Goal(s) for time on the South West London ICL Digital Pioneer Fellowship: Enhance cross-sector collaboration and leadership to create scalable, sustainable digital solutions that align with K&R priorities, driving measurable improvements in community health and well-being.
